REU Summer Intern Paperwork
Prior to your arrival, you must complete the following paperwork. A delay in paperwork can result in a delay of your stipend payment or receiving your HUID, which can prohibit access to the dorms, labs, and WIFI.

You will be asked to enter some basic information, and you will need to sign and upload the following documents:

Offer Letter - please read, sign, and upload the PDF offer letter sent to you with the paperwork instructions email

Visitor Participation Agreement - please read, sign, and upload

Direct Deposit Form - (For US Bank accounts ONLY) - please fill out and upload if you would like to use Direct Deposit for your stipend payments. We highly recommend setting up Direct Deposit before you arrive. Please note that if you choose to not use Direct Deposit or your information is entered late, your checks will be mailed to Kathryn's office and will be distributed as they are received. This might result in a delay in receiving your check.

Code of Conduct - please read, sign, and upload

Proof of Health Insurance - please scan (or photograph) the front and back of your health insurance card and upload

Photo - please upload a photo of yourself for our REU Participant Profile page

Bio - Write a short bio about yourself to be used on our website at the end of the summer. Examples can be found here: https://sites.google.com/g.harvard.edu/reu-participant-profiles/home
Please note - while we no longer collect immunization forms, you must be up to date on your immunizations to live in on-campus housing. In case of a health emergency you must be able to quickly access your immunization information. Please bring a copy of your records (digital or printed) with you.

Other considerations
Students are eligible for up to $450 for travel reimbursement for travel to/from the REU program.

You may only be reimbursed for domestic, economy fare (airfare, bus, or train).

You must submit travel receipts which show the amount paid and method of payment.
